Additional Re-Enrollment Forms

The following forms must be submitted online to ensure that your student is re-enrolled. Please click on each applicable link to submit your forms. Failure to complete any form listed will leave your student’s spot open and available to other prospective students.

1. Family Handbook Acknowledgment (all students)

2. Enter your Scholarship Award ID (all students receiving scholarship funding)

3. Student Wellness Profile (all students)

4. Out of School (OOS) Registration Packet (any student who will utilize OOS services; please download, complete, and submit in the front office)

5. Early Childhood Application Packet (all students in Pre-K2 to VPK)

6. Promotional Criteria Acknowledgment (all students entering 5th or 8th grade)

7. Chromebook Contract with Acceptable Use Policy (all middle school students)
